When you click on the "Collaborate" button on the front end, the contact form popup pops up three times. This is evident when you close the popup and there are still popups behind it that need to be closed also.

It appears that you've disabled LayerSlider in the meantime, so I wasn't able to debug on your site unfortunately.
These issues usually occur if you're also using the auto-include feature that might include duplicate copies of the slider without you realising that. Another explanation would be your theme. Some themes tend to re-append content in the document, which is considered a really bad practise and causes issues exactly like this. You can easily test this if you temporarily switch to one of the default WP themes.
I've checked your site again, but it seems that LayerSlider is still inactive. If you're using some kind of caching plugin, please make sure to empty the caches in order to avoid issues like this.

Unlike conventional responsive behavior, it duplicates the content (including the slider) and dynamically switch to the appropriate version depending on the device and screen size.
You can likely workaround this behavior by not using shortcodes and other conventional methods, instead, let LayerSlider to automatically include the slider. This is the auto include feature I mentioned earlier. You can learn more about using it by reading our online documentation.
In short, you can just tell LayerSlider on which pages it should include your Popup in the Slider Settings -> Popup -> Target Pages section and you're all set. No shortcodes or widgets required. In fact, you should remove the shortcode or anything you are currently using.
I've double checked and can't see any shortcodes for the popup in the page, just the post-id number specified in the popup target pages - 116848, 122818, 121748 (landing page, home page and aboutus page).
I've also tried selecting "prevent reopening" but this also seems to make no difference. In case it's of assistance, I included logon details for the admin panel in a previous private email which was part of this chain of conversations.

However, those blocks do not appear on the About page, and try as I might, I have been unable to locate any openpopup shortcodes on this particular page - ironically, the only page that has the popup duplication issue?
